Community Health Partnership of Illinois (CHP) is a private 501(c)(3) not-for-profit organization in Illinois dedicated to serving its communities with primary preventative medical, dental, and behavioral health services. CHP has been in existence for over 50 years and since 1970. Our employees and providers provide services to more than 10,000 patients from 6 counties across central and northern Illinois.
Under general supervision of the Nursing Director, the lead nurse provides and coordinate patient care, educates patients and the public about various health conditions, and provide advice and emotional support to patients and their families. The nurse works closely with supervising physicians to assess and monitor patients' needs, ensure treatment an care is conducted according to the procedures of the facility, administer medications, perform other related task and in collaboration with clinical teams, providers, management team, the Lead Nurse delegates nursing assignments, prepares schedules, oversees patient intake and discharge, monitor and order medicines and supplies for his/her assigned clinic(s). Other responsibilities include leading site clinical programs, supporting other clinics, onboarding new nurses, caring for patients, supervise nursing staff, offer guidance and support, maintain a clean and safe working environment, and ensure all operational and clinical regulatory tasks are completed efficiently, in a compliant way and on time.
Primary Responsibilities:
- Supervise assigned clinic staff (MAs/CSAs).
- Direct and coach other nurses on patient care, providing guidance and advice and supporting other nurses as needed within CHP.
- Ensure patient referral and tracking are accomplished and patient files are updated with results from referrals and results to include specialty and lab results.
- Assure quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and hospital or regulatory standards.
- Resolve patient problems and needs by utilizing multidisciplinary team strategies. Maintain a safe and clean working environment by complying with procedures, rules, and regulations.
- Protect patients and employees by adhering to infection-control policies and protocols; medication administration and storage procedures; and controlled substance regulations.
- Supporting the Nursing Director on needed clinical, new and existing clinical initiatives.
- Support CHP's clinic gap, inclusive of much needed clinical policies and protocols.
- Support the Nursing Director on the development and implementation of much needed clinical programs to provides added value to our patients, these programs may include but not limited to; Diabetes programming, MAT, and other clinical patient education.
- Provide support at various CHP sites as needed.
Required Licenses & Certifications:
- Valid Illinois nursing license (LPN or RN)
- CPR certification
- Medication Assisted Treatment Waiver (MAT} trained or Interest in MAT treatment of substance use disorders.
- Annual OSHA/HIPPA training in accordance with the CHP OSHA/HIPPA manual.
- Bachelor's Degree in Nursing or more advance Nursing degree.
- Proficient in Microsoft Office and medical EMR software.
