Community Health Partnership of Illinois is a is a non-profit organization that provides primary health care to migrant farmworkers and other medically underserved populations throughout northern and central Illinois that specializes in primary preventative medical, dental, and behavioral health service. Our focus is delivering quality, culturally responsive health care services to our patients, caregivers, and partners in the communities we serve.

The Outreach Nurse provides primary medical and complete progress notes and assessments in the electronic health record within a timely manner. Provide services to consumers based on current risk and status. Work with external stakeholders – such as hospitals, farms,  public schools, shelters, jails, primary care – to achieve positive client outcomes. 

Description of Primary Responsibilities

  • The ability to meet, engage vulnerable people in various settings and life circumstances.
  • Sensitivity to emotional cues of others; respond appropriately; interview in a non-threatening manner.
  • Inclusive, positive attitude toward vulnerable people

Qualification:

  • RN degree from an accredited school of nursing
  •  Advanced nursing degree from an accredited school or college of nursing.

License/Certifications

  • Current License as a registered nurse, Illinois
  • Advanced practice registered nurse (APRN) license, Illinois
  • Approved APRN board certification
  • BLS Certification
  • DEA License
