Family Nurse Practitioner (FNP)
Community Health Partnership of Illinois (CHP) is seeking a compassionate and patient-centered Family Nurse Practitioner (FNP) to join our team in Aurora, Illinois. In this role, you will provide high-quality primary care to patients across the lifespan while supporting CHP’s commitment to accessible, culturally responsive healthcare.
Community Health Partnership of Illinois is a 501(c)(3) not-for-profit organization that provides primary and preventive healthcare services to uninsured and underserved communities throughout northern and central Illinois. CHP provides access to medical, dental, and behavioral health services, as well as connections to other social services.
Responsibilities
- Provide primary care to patients of all ages, from infants to older adults.
- Conduct physical examinations and diagnose and treat common illnesses and injuries.
- Order and interpret diagnostic tests and prescribe medications as appropriate.
- Manage acute and chronic health conditions and provide preventive care and health education.
- Refer patients to specialists when necessary and collaborate with other healthcare providers to ensure coordinated care.
- Maintain accurate and timely patient documentation.
- Participate in quality improvement initiatives and support established clinical policies and practices.
Qualifications
- Master’s or doctoral degree in Nursing.
- Current Illinois RN and APRN licensure.
- Certification as a Family Nurse Practitioner.
- Current CPR/BLS certification.
- Previous primary care or related clinical experience preferred.
- Strong communication, interpersonal, and patient care skills.
- Bilingual/Spanish preferred, but not required.
