Nurse Practitioner

Community Health Partnership of Illinois (CHP) is seeking a compassionate and dedicated Nurse Practitioner (NP) to join our healthcare team. The Nurse Practitioner will provide comprehensive, patient-centered primary care services to individuals and families while supporting CHP’s mission of providing quality healthcare to the communities we serve.

Responsibilities

• Provide comprehensive primary care services to patients across the lifespan.
• Conduct physical examinations, health assessments, and routine preventive care.
• Diagnose and manage acute and chronic health conditions.
• Order, perform, and interpret appropriate diagnostic and laboratory tests.
• Develop and implement individualized treatment and care plans.
• Prescribe medications and treatments within the scope of practice.
• Educate patients regarding diagnoses, medications, preventive care, and healthy lifestyle practices.
• Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
• Collaborate with physicians, nurses, clinical staff, and other healthcare professionals to coordinate patient care.
• Make appropriate referrals to specialists and community resources when necessary.
• Follow CHP clinical policies, procedures, quality standards, and regulatory requirements.
• Participate in quality improvement and population health initiatives.
• Perform other related duties as assigned.

Qualifications

• Master’s or Doctoral degree from an accredited Nurse Practitioner program.
• Current and unrestricted Illinois Advanced Practice Registered Nurse (APRN) license or eligibility to obtain one.
• Current national Nurse Practitioner certification from a recognized certifying organization.
• Current Registered Nurse (RN) license in Illinois, as required.
• Current DEA registration or eligibility to obtain registration, if prescribing controlled substances is required for the position.
• Current BLS/CPR certification.
• Strong clinical assessment, diagnostic, and patient-care skills.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work effectively with patients from diverse cultural and socioeconomic backgrounds.
• Experience in primary care, community health, or a Federally Qualified Health Center (FQHC) environment is preferred.
• Bilingual skills are a plus.
